This 60 sheet score pad is a tool to help you give better feedback to a designer, or to get better feedback from your play testers.
It includes quick check boxes to easily articulate the things that are holding back a game's design. It also has a feedback form to leave more detailed feedback including the great things about the game.

This tool can be used in a lot of different ways, but I thought I'd tell you why I designed it and how I intend to use it.
I will be filling this out myself to give to designers when I play their games. However, this will not be the only bit of feedback that I provide. As is traditional, we'll have a discussion after a play test, which will be the primary feedback. I'll be using this tool to take quick notes during the play test (hence all the check boxes, no time to write). These notes will then remind me of things to discuss during the play test discussion. I'll still give the designer these notes, so that they'll have them and do what they want, but ultimately it is inside the context of that discussion we've had. They won't "just" get this sheet.
